It’s caused by memory leak and performance issue with a very old hand made npm module (by myself): listag.
In order to identify this issue, we upgrade the Heroku Dyno plan from free to professional ($25/m) so that we can get the Metrics.
After removed the buggy module listag and refactored the server code, everything works like a charm.
